Terms of Sale
First Response Carpet Care terms of sale are net 30 days from date of invoice. All invoices not paid in a timely manner may cause interruption of credit extension unless arrangements are made with the credit department. First Response Carpet Care will automatically assess a 18% finance charge per month on all past due invoices. Applicant agrees to pay any collection costs incurred to collect the account balance, including reasonable attorney's fees.

Applicant's Statement
Applicant authorizes First Response Carpet Care to verify or check any of the information given, check credit references, and obtain one or more credit reports in connection with this credit application or in connection with any periodic reviews of credit which may be extended to Applicant. Applicant gives all of Applicant's creditors banking relationships permission to give First Response Carpet Care any information it needs to make a credit granting decision. Applicant gives First Response Carpet Care  permission to report to credit reporting agencies and others the results of such investigations and First Response Carpet Care  experience with Applicant's account. Applicant agrees that exclusive jurisdiction and venue shall be in the state or federal courts in Tarrant County, Texas.
